Job summary

Local Infusion is seeking a Nurse Practitioner to provide telehealth neurologic and memory-care care in a remote, patient-facing role guided by a supervising physician. You will assess patients, create care plans, and manage ongoing treatment with prescription authority and detailed EMR documentation.

You will educate patients and families on diagnoses, treatment options, and disease progression, ensuring continuity of care with the broader team.

Qualifications

• Licensed NP in New Hampshire with current credentials.
• Telehealth clinical experience preferred.
• Ability to collaborate with physicians and care team.
• Strong patient and caregiver communication.

Role Overview: Nurse Practitioner, Neurology and Memory Care

We are seeking a skilled and compassionate Nurse Practitioner who is licensed in New Hampshire to join our virtual care team. This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.

Job Description

• Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies
• Partner with our neurologist(s) and broader care team to ensure continuity of care
• Educate patients and families on diagnoses, treatment plans, and disease progression
• Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements

Key Accountabilities

• Active, unrestricted NP license in New Hampshire; certification in geriatrics, adult-gerontology, or family practice preferred
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
• Strong clinical judgement and clinical documentation skills
• Comfortable managing a full patient panel independently with physician collaboration
• Excellent communication skills with patients facing cognitive decline and their caregivers
