Local Infusion is seeking a licensed Nurse Practitioner in New Hampshire for a remote, patient-facing neurology and memory care role. You will provide telehealth assessments, develop care plans, and prescribe therapies in collaboration with supervising physicians.

Qualifications

• Active, unrestricted NP license in New Hampshire.
• Certification in geriatrics, adult-gerontology, or family practice preferred.
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings.
• Strong clinical judgement and clinical documentation skills.
• Excellent communication with patients facing cognitive decline and caregivers.
• Comfortable managing a full patient panel independently with physician collaboration.

Responsibilities

• Provide direct clinical care to patients with neurological conditions via telehealth.
• Conduct patient assessments, develop care plans, and manage ongoing treatment with supervising physicians including prescribing therapies.
• Partner with neurologists and the care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatment plans, and disease progression.
• Maintain accurate, timely clinical documentation in the EMR in line with regulatory requirements.

We are seeking a skilled and compassionate Nurse Practitioner who is licensed in New Hampshire to join our virtual care team. This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.

• Partner with our neurologist(s) and broader care team to ensure continuity of care
• Educate patients and families on diagnoses, treatment plans, and disease progression
• Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements
