Local Infusion is a remote-first team delivering infusion care with a neurology focus. We seek a licensed Nurse Practitioner in Maine to join our virtual care program, providing direct telehealth services to patients with neurological conditions and memory disorders, collaborating closely with physicians to expand access.

In this full-time role you will assess patients, develop care plans, educate families, document in the EMR, and ensure continuity of care with the care team.

Qualifications

• Licensed Nurse Practitioner in Maine.
• 2+ years in neurology or memory care settings.
• Strong clinical judgement and documentation skills.
• Ability to manage a full patient panel with physician collaboration.
• Excellent communication with patients and caregivers.

Responsibilities

Role Overview: Nurse Practitioner, Neurology and Memory Care

We are seeking a skilled and compassionate Nurse Practitioner who is licensed in Maine to join our virtual care team. This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.

Job Description

• Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies
• Partner with our neurologist(s) and broader care team to ensure continuity of care
• Educate patients and families on diagnoses, treatment plans, and disease progression
• Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements

Key Accountabilities

• Active, unrestricted NP license in Maine; certification in geriatrics, adult-gerontology, or family practice preferred
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
• Strong clinical judgement and clinical documentation skills
• Comfortable managing a full patient panel independently with physician collaboration
• Excellent communication skills with patients facing cognitive decline and their caregivers
