Job summary

Local Infusion is seeking a licensed Nurse Practitioner (NH) to join our remote-first neurology and memory care team. This patient-facing, telehealth role collaborates with physicians to improve access and timeliness of treatment for neurological conditions.

The NP will conduct assessments, develop care plans, prescribe therapies as allowed, educate patients and families, and maintain compliant EMR documentation. Remote work supports work-life balance while delivering high-quality care.

Qualifications

• Active, unrestricted NP license in New Hampshire.
• Certification in geriatrics, adult-gerontology, or family practice preferred.
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings.
• Strong clinical judgement and clinical documentation skills.
• Comfortable managing a full patient panel independently with physician collaboration.
• Excellent communication skills with patients facing cognitive decline and their caregivers.
