Job summary

Local Infusion is seeking a skilled Nurse Practitioner licensed in Connecticut for a remote, patient-facing neurology and memory care role. You will deliver care via telehealth to Local Infusion patients and collaborate closely with physicians and the care team to improve access and reduce time to treatment.

In this full-time telehealth position, you will conduct assessments, develop care plans, prescribe therapies in collaboration with supervising physicians, and educate patients and families

Qualifications

• Active, unrestricted NP license in Connecticut; geriatrics/memory care preferred.
• 2+ years of clinical experience in neurology/memory care or dementia settings.
• Strong clinical judgement and accurate documentation skills.
• Ability to manage a full patient panel with physician collaboration.
• Excellent communication with patients and caregivers facing cognitive decline.

We are seeking a skilled and compassionate Nurse Practitioner who is licensed in Connecticut to join our virtual care team. This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.

Key Accountabilities

• Active, unrestricted NP license in Connecticut; certification in geriatrics, adult-gerontology, or family practice preferred
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
• Strong clinical judgement and clinical documentation skills
• Comfortable managing a full patient panel independently with physician collaboration
• Excellent communication skills with patients facing cognitive decline and their caregivers
