Job summary

My Local Infusion is seeking a skilled Nurse Practitioner licensed in Connecticut to join our remote care team. This patient-facing role delivers telehealth neurologic and memory care services to Local Infusion patients, collaborating with physicians to improve access and timing of treatment.

You will conduct assessments, develop care plans, prescribe therapies under supervision, educate patients and families, and document in the EMR in compliance with regulations.

Qualifications

• Active, unrestricted NP license in Connecticut.
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings.
• Strong clinical judgement and clinical documentation skills.
• Comfortable managing a full patient panel independently with physician collaboration.
• Excellent communication skills with patients facing cognitive decline and their caregivers.

We are seeking a skilled and compassionate Nurse Practitioner who is licensed in Connecticut to join our virtual care team. This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.
