Job summary

ChristianaCare’s Swank Center for Memory Care and Geriatric Consultation seeks a Memory Care Specialist APC to work virtually as a full-time clinician. You will triage, coordinate care, and support families via Epic inbox, delivering expert guidance between visits.

The role emphasizes dementia care, collaboration with a multidisciplinary team, and ongoing program development with substantial CMA education support. Remote after onboarding, with initial onsite training in Wilmington, DE.

Qualifications

• Must hold an NP or PA degree with board certification in an applicable specialty.
• Delaware licensure or eligibility and DEA/CSR compliance required.
• Fellowship training or 3+ years geriatrics or related specialty preferred.

Responsibilities

• Manage patient, caregiver, and provider communications via Epic In Basket.
• Evaluate and triage memory disorder-related clinical concerns.
• Conduct virtual visits and follow-up care between appointments.
• Review diagnostics, manage medications, and coordinate treatment.
• Educate patients/caregivers and connect to community resources.
• Collaborate with physicians, APCs, nurses, social workers, and team.
• Provide coverage during provider absences and scheduling gaps.
• Support access, quality, workflows, and continuity of care initiatives.

Education

Master’s or Doctoral degree from NP/PA program

Board Certification: FNP/AGNP/AGPCNP/PA-C/PMHNP

Delaware APRN or PA license, or eligibility

DEA registration and CSR

BLS certification

Fellowship or 3+ years geriatrics experience
