Job summary

ChristianaCare’s Swank Center for Memory Care and Geriatric Consultation seeks an experienced Nurse Practitioner or Physician Assistant as a full-time Memory Care Specialist APC. The role combines virtual patient care, triage, caregiver support, care coordination, and population management to enhance access for patients with memory disorders.

Qualifications

• Master's or Doctoral degree from an accredited NP/PA program.
• Board Certification in FNP, AGNP/AGPCNP, PA-C, or PMHNP with relevant experience.
• Current Delaware APRN or PA license or eligibility; DEA and CSR; BLS.
• Fellowship training or 3+ years in geriatrics/dementia care is preferred.

Responsibilities

• Manage communications via Epic In Basket with patients, caregivers, and providers.
• Triage urgent and semi-urgent memory/neurocognitive concerns.
• Conduct virtual visits and provide follow-up care between visits.
• Review diagnostic results, manage medications, and coordinate treatment recommendations.
• Educate patients and caregivers and connect to community resources.
• Collaborate with physicians, APCs, nurses, social workers and the care team.
• Support coverage during provider absences and scheduling gaps.
• Participate in initiatives to improve access, quality, and continuity of care.
