Job summary

ChristianaCare’s Swank Center for Memory Care and Geriatric Consultation seeks an experienced Nurse Practitioner or Physician Assistant for a full-time, primarily remote Memory Care Specialist APC role. You will manage Epic In Basket communications, triage memory-related concerns, and conduct virtual visits with families and patients.

The ideal candidate has a master’s/doctoral degree in NP/PA, relevant board certs, and licensure in Delaware or eligibility.

Qualifications

Master's or Doctoral degree from an accredited NP/PA program.
Board Certification: FNP, AGNP, AGPCNP, PA-C, or PMHNP with relevant experience.
Current Delaware APRN or PA license (or eligibility).
DEA registration and Delaware CSR.
BLS certification.
Fellowship training or 3+ years in geriatrics, dementia care, or related specialty.

Responsibilities

Manage patient, caregiver, and provider communications via Epic In Basket.
Triage urgent and semi-urgent memory-related concerns.
Conduct virtual visits and follow-up care between scheduled appointments.
Review diagnostic results and coordinate treatment recommendations.
Educate patients and caregivers and connect to community resources.
Collaborate with physicians, APCs, nurses, social workers and other team members.
Provide clinical coverage during provider absences and scheduling gaps.
Participate in initiatives to improve access, quality, workflows, and continuity of care.
