After onsite onboarding in Wilmington, DE, the position is primarily remote with a Monday–Friday schedule and no evenings or weekends. You will help shape an evolving memory care program while collaborating with a multidisciplinary

Qualifications

• Master's or Doctoral degree from an accredited NP/PA program.
• Board Certification as FNP/AGNP/AGPCNP/PA-C/PMHNP with relevant experience.
• Current Delaware APRN or PA license, or eligibility for Delaware licensure.
• DEA registration and Delaware CSR.
• BLS certification.
• Fellowship/advanced specialty training or 3+ years in geriatrics or dementia care.

Responsibilities

• Manage communications through Epic In Basket.
• Evaluate and triage urgent clinical concerns related to memory disorders.
• Conduct virtual visits and follow-up care between visits.
• Review diagnostics, manage medications, and coordinate treatment.
• Educate and support patients and caregivers; connect to resources.
• Collaborate with physicians, APCs, nurses, social workers, and others.
• Provide coverage during provider absences and scheduling gaps.
• Help improve access, quality, and continuity of care at the Swank Center.
