Placements24 is seeking a Remote Care Coordinator to support community well-being from a fully remote role based in South Africa. You will coordinate client care plans, conduct virtual assessments, and liaise with families, caregivers and healthcare professionals using secure case management tools.

The ideal candidate has a relevant qualification in Social Work or a related field, at least three years in community support, and excellent written and verbal communication.

Qualifications

• Relevant qualification in Social Work, Community Health, Psychology, or related field.
• Minimum of 3 years of experience in community support, social work, or care coordination.
• Proven experience in managing cases and coordinating services.
• Excellent proficiency with remote work technologies and online communications.

Responsibilities

• Coordinate and manage client care plans remotely, ensuring needs are addressed.
• Conduct virtual client assessments and follow-ups to monitor progress.
• Facilitate communication between clients, families, healthcare providers, and support staff.
• Utilize case management software to maintain accurate client records.
• Provide information and referrals to community resources and support services.
• Troubleshoot and resolve issues in the delivery of remote care services.
• Develop and implement efficient remote support protocols.
• Ensure compliance with privacy regulations and organizational policies.
• Participate in virtual team meetings and contribute to continuous improvement.
• Offer empathetic support and guidance to clients navigating complex care needs.
