Job summary

Safe Place in Lake Forest, IL is seeking a Transitional Housing Case Manager to help survivors of domestic violence secure transitional housing and coordinate TH RRH services, promoting empowerment and self-sufficiency through rental assistance and landlord partnerships.

You will conduct VISPDAT assessments, manage intake and eligibility, and connect clients with employment, education, counseling, and legal advocacy.

Qualifications

• Bachelor's degree in Social Work or related field is required.
• Flexible schedule to meet client needs and crises.
• Bilingual preferred.
• Ability to work independently and as part of a team.
• Strong written and oral communication skills.
• Knowledge of maintaining confidential information.

Responsibilities

• Develop partnerships with landlords.
• Educate landlords about policies to keep families safe.
• Provide information on the Illinois Domestic Violence Act and related laws.
• Manage applications, updates, and recertifications for housing.
• Oversee temporary housing for two families in ASP units.
• Outreach to community organizations and landlord associations.
• Coordinate intake assessments and eligibility evaluations.
• Complete housing assessments and VISPDAT evaluations.
• Create service plans and coordinate services (employment, education, counseling, etc.).
• Ensure documentation in client records is accurate and timely.
• On-call rotating schedule (7 days) including one weekend every 4–6 weeks.
• Attend coordinated entry meetings and team gatherings.
• Collaborate with ASP staff and refer additional services.
• Develop relationships with partners and provide trainings.
• Perform additional responsibilities as assigned.
