Job summary

TASC seeks a full-time Case Manager for the Community Reentry program, delivering addiction intervention, AODA counseling, and recovery support within Illinois. You will coordinate services, collaborate with partners, and help pregnant and post-partum clients and families achieve set goals.

Role requires working across correctional and community settings, maintaining confidentiality, and presenting strong organizational and communication skills. Valid driver's license preferred.

Qualifications

• High school diploma or GED required.
• Demonstrated proficiency in Microsoft Office suite.
• Demonstrated experience in group facilitation.
• Good verbal, written, and interpersonal communication skills.
• Good time management skills with ability to meet deadlines.

Responsibilities

• Assist individuals in achieving service plan goals through direct services and coordination with health, public agencies, and community resources.
• Assess, evaluate, facilitate, and monitor progress with contractual benchmarks.
• Complete intake packets for program enrollment and participation.
• Provide referrals and crisis intervention upon release from prison.
• Interview individuals and review records to identify reentry needs.
• Collaborate with partners, attend staff meetings, and participate in integrated staffing.
• Refer individuals to initial treatment and behavioral health services.
• Complete and maintain documentation and program reports.
