Qualifications

• Bachelor’s degree in social work or related field preferred.
• Experience in case management or social services.
• Strong communication and relationship-building skills.
• Knowledge of community resources and service access.
• Ability to assess needs, develop plans, and monitor progress.
• Familiarity with mental health, substance abuse, domestic violence, and criminal justice issues.
• Record-keeping and case documentation proficient.
• Ability to work independently and with a team.

Responsibilities

• Conduct intake assessments to identify client needs.
• Develop tailored case plans to guide clients in achieving their goals.
• Provide or facilitate access to services including therapies, legal aid, housing, training, and re-entry.
• Build trust with clients through effective communication.
• Maintain precise intake forms and detailed case records.
• Collaborate with workforce development programs for timely opportunities.
• Manage a caseload of up to 25 participants.
• Contribute to gun violence prevention and community safety.
• Connect participants with workforce programs.
• Ensure services are offered only to eligible participants.
