Veterans' Safe Haven Case Manager – Day Shift

Job summary

Paycom is seeking a Safe Haven Day Case Manager in Boston to support a 25-bed, 24/7 Veterans program experiencing homelessness with co-occurring disorders. You will provide clinical case management and targeted counseling, applying Motivational Interviewing, Harm Reduction, Relapse Prevention and Trauma Informed Care to residents.

The role requires a Bachelor's degree in a human services field and 3–5 years in case management, with preference for LADC/CADC and veteran status.

Qualifications

• Bachelor's degree in human services-related field required.
• 3–5 years of case management experience with substance use disorders and mental illness.
• Knowledge of housing first, harm reduction, Motivational Interviewing and trauma-informed care.
• Crisis intervention and de-escalation skills.
• Strong communication and interpersonal abilities.

Responsibilities

• Maintain a caseload of Veterans and connect to services.
• Provide individual supportive counseling using evidence-based approaches.
• Collaborate with VA Liaison and Center staff to coordinate care.
• Ensure data collection, documentation deadlines and progress notes.
• Support housing placement, income, benefits and discharge planning.
• Attend department and program meetings and perform other duties as assigned.
