Job summary

Northeast Kingdom Human Services is seeking a part-time Veterans Outreach Case Manager in St. Johnsbury, VT. The role focuses on outreach, home visits, and care coordination for veterans and military personnel within a comprehensive mental health program.

Candidates should have a bachelor’s degree in human services or related field, experience with VA health systems, and a commitment to serving diverse, low-income communities. Travel between Derby and St. Johnsbury is required.

Qualifications

• Bachelor’s degree in human services or related field preferred.
• Experience with veteran outreach and establishing rapport with service members.
• Knowledge of the VA health system highly preferred.
• Experience with diverse, low-income populations and behavioral health.

Responsibilities

• Assists therapists and medical team in meeting veteran needs.
• Provides home visits, outreach and follow-ups to engage clients in care.
• Develops partnerships with VA centers and community clinics.
• Ensures integrated care across substance use and mental health services.
• Assigns each veteran a Principal Behavioral Health Provider.
