Job summary

HomeFirst is seeking an experienced Case Manager for the SSVF program in Santa Clara County. This on‑site position supports participants in housing stability plans, conducts assessments, and assists with budgeting and benefits to reduce barriers to housing.

You will maintain a caseload, document services, and collaborate with the Program Manager to ensure high‑quality, compliant client support and outcomes in a team‑oriented environment.

Qualifications

• 1 year of paid or volunteer experience in social work or related human services, required
• Bachelor of Arts in Social Work (BASW) or related human services degree, preferred, not required
• Proficient in Microsoft Office suite
• Valid CA driver’s license and insurance, and ability to be added onto agency insurance

Responsibilities

• Maintain a caseload of 20-30 participants.
• Provide comprehensive case management services including eligibility, verification and enrollment.
• Ongoing needs and status assessments.
• Budgeting and administration of financial assistance.
• Information and referral, crisis de‑escalation, safety planning and risk assessment.
• Employment and benefits assistance, advocacy for participants.
• Document thoroughly in accordance with agency standards.
• Attend meetings and participate in learning opportunities.
