Job summary

HomeFirst in San Jose, CA is seeking a Temp Case Manager for the Supportive Services for Veteran Families (SSVF) program in Santa Clara County. The role focuses on housing stability planning and connecting veterans and their families to essential services.

This temporary, full-time position (4-5 months) involves direct case management, documentation in HMIS, collaboration with team members, and coordinating outreach across sites.

Qualifications

• 2 years paid or volunteer experience in social services (required).
• Bachelor of Arts in Social Work (BASW) or related degree preferred, not required.
• Bilingual Spanish or Vietnamese speaking or bicultural preferred.
• Strong written and oral communication skills.
• Ability to manage multiple projects concurrently and meet deadlines.
• Understanding of and sensitivity to the needs of the homeless population, or formerly homeless.

Responsibilities

• Direct Services (70%): provide case management and direct services to clients in Santa Clara County; coordinate with team and community partners.
• Documentation (20%): maintain HMIS records and complete progress notes within three business days; manage incident reports and contracts.
• General (10%): attend meetings/trainings; cover across program sites; uphold agency policies and procedures.
