Job summary

HomeFirst is seeking a Temp Case Manager for the Santa Clara County program. The role focuses on housing stability planning, client advocacy, and connecting participants with essential services across county locations.

The position is temporary, full-time/non-exempt for 4-5 months, with a day shift and up to 25% travel. Compensation is $33.17 per hour, with benefits typical for nonprofit employers.

Qualifications

• 2 years paid or volunteer experience in social service or related human services (required).
• Bachelor of Arts in Social Work (BASW) or related human services degree preferred, not required.

Responsibilities

• Provide case management and direct services to clients throughout Santa Clara County.
• Deliver community-based case management services tailored to each client’s needs.
• Coordinate with team members remotely and on-site to communicate interventions and link clients to services.
• Collaborate with community partners to ensure access to support services for clients.
• Maintain HMIS documentation within three business days and ensure compliance with grant contracts.
• Assist clients with transportation to critical appointments using company vehicles when assigned.
