You will connect Veterans with housing resources, coordinate outreach, and support stability after placement. The role emphasizes collaboration with shelters, agencies, and community resources, while maintaining accurate records and ensuring confidentiality.

Qualifications

• Bachelor’s degree in social services or related field preferred.
• At least two years of experience in case management, life-skills support, assessments, counseling, housing services, outreach.
• Experience serving Veterans preferred.
• Knowledge of housing requirements, landlord engagement, and community resources.
• Ability to communicate clearly and professionally with stakeholders.

Responsibilities

• Build a strong network of housing partners and maintain landlord relationships.
• Inspect units to ensure HUD/VA Housing Habitability Standards and TIP program requirements are met.
• Connect Veterans with the TIP program and manage referral pathways.
• Coordinate targeted community outreach with partner organizations.
• Support housing stability after placement with follow-up and issue resolution.
• Maintain complete housing records, inspections, and outreach documentation.
