Job summary

You will coordinate housing services, work with landlords and community partners, conduct outreach, and help participants secure housing while maintaining HMIS documentation.

Qualifications

• Bachelor's degree in a related field or 2 years work experience in social services.
• Ability to collaborate with landlords, property managers, and agencies to support housing stability.
• Knowledge of mental health, substance use, crises, and veteran services.

Responsibilities

• Conduct outreach and assess veterans to determine eligibility for services.
• Identify and coordinate delivery of housing services with partners.
• Provide services through home visits, office appointments, and community visits.
• Coordinate with VA Medical Center staff and HUD-VASH partners.
• Maintain participant files and HMIS documentation.
