Qualifications

• Minimum of 3 years of case management experience.
• Ability to travel via public transit to engage veterans up to 40% of the work week.
• Ability to communicate effectively with participants, team members, and stakeholders.

Responsibilities

• Provide resources and referrals to help participants obtain safe, permanent housing.
• Navigate veterans through VA services, public benefits, and housing resources.
• Deliver person-centered case management using progressive engagement to support housing goals.
• Develop and monitor Housing Stability Plans with SSVF staff and programs.
• Meet participants in the field, VA sites, shelters or residences as needed.
