Job summary

Catholic Community Services is seeking a Case Manager for its Veterans Services program in the Bremerton area. You will manage a caseload, conduct intakes, assess housing needs, and coordinate financial assistance to stabilize housing for Veterans and their families.

Responsibilities include developing Housing Stability Plans, coordinating with VA resources, conducting outreach, and maintaining HMIS records to support program goals and timely services.

Qualifications

• Post-secondary education in social services field or 2 years relevant experience.
• Excellent interpersonal and written/oral communication skills.
• Strong organizational and time management abilities.
• Proficiency with Microsoft Word and database systems.

Responsibilities

• Complete thorough intakes and eligibility screening.
• Develop and update Housing Stability Plans with clients and VA/community resources.
• Provide outreach to eligible Veterans and generate referrals.
• Enter initial information into HMIS and maintain service transactions.
• Maintain accurate case notes and 90-day recertifications as applicable.
• Attend meetings and represent CCS Veterans Services.
