Job summary

Catholic Community Services of Western Washington is seeking a Case Manager for the Veterans Services program across nine counties. You will manage a caseload in the Homeless Prevention/Rapid Re-housing initiative, conduct intakes, assess housing needs, and provide timely financial assistance to help families secure stable housing.

Collaborate with the Veterans Program Manager, develop Housing Stability Plans, enter data into HMIS, and coordinate with VA and community resources to ensure timely,

Qualifications

• Post-secondary education in social services or 2 years relevant experience.
• Experience providing services to low-income or no-income households.
• Experience in case management and strong written/spoken communication.
• Reliable transportation, valid driver's license and insurable vehicle.
• Ability to pass background checks and obtain CPR/First Aid training.

Responsibilities

• Complete thorough intakes and eligibility screenings with referred clients.
• Initiate Housing Stability Plan and prioritize top housing needs.
• Provide outreach to Veterans and generate referrals to targeted services.
• Enter initial and ongoing service transactions into HMIS and maintain records.
• Attend meetings and coordinate with the Veterans Services Program and partners.
