Job summary

Qualifications

• Post-secondary education in social services or 2 years relevant experience.
• Experience providing services to low-income/no-income households.
• Experience in case management.
• Excellent interpersonal and communication skills.
• Strong organizational and time management skills.
• Ability to work in a team and promote positive team spirit.

Responsibilities

• Complete thorough intakes and eligibility screening with referred clients.
• Initiate Housing Stability Plan and coordinate interventions.
• Provide outreach to eligible Veterans and generate referrals.
• Enter initial and ongoing service data into HMIS.
• Update and carry out client Housing Stability Plan with VA/community resources.
• Maintain accurate records, notes, and reports.
• Attend meetings and participate in training workshops.
