Job summary

Volunteers of America Chesapeake & Carolinas seeks a Case Manager to provide day-to-day support for participating veteran households in Raleigh, NC. Responsibilities include intake through to housing attainment, outreach, and wraparound services in coordination with VA and community partners.

The role requires a bachelor's degree in a related field and experience with homelessness prevention or veterans services.

Qualifications

• A bachelor's degree in counseling psychology, rehabilitative counseling, or related human service field is preferred (or comparable experience).
• Experience working with homeless individuals and families including veterans is a plus.

Responsibilities

• Coordinate outreach efforts for the SSVF program with VA and housing partners.
• Screen program applicants for eligibility using VA-approved forms and verify information with applicable agencies.
• Assess housing and comprehensive supportive services needs and develop a Family Services Plan.
• Provide service coordination between the program and community partners.
• Collaborate with clients, family members, housing agencies and professionals to revise service plans and placement.
• Arrange housing support and wraparound services to support transition to permanent housing.
