Job summary

Volunteers of America Chesapeake and Carolinas is seeking a Case Manager in Durham, NC to support the SSVF program. You will provide day‑to‑day case management for participating veteran households, coordinating homelessness prevention, rapid re‑housing, and wraparound services with VA and community partners.

The role focuses on intake, assessment, service planning, and linkage to housing resources, with travel within the region as needed.

Qualifications

• Bachelor’s degree in counseling psychology, rehabilitative counseling, or related human services field (or comparable experience).
• Experience working with homeless individuals and families, including veterans, is a plus.

Responsibilities

• Coordinate outreach efforts for the SSVF Program including establishing an outreach plan for the three participant categories, scheduling visits to the VA, shelters, and locations where homeless and low‑income veteran families congregate, and communicating with the VA and key housing and community providers.
• Screen potential program participants for eligibility using VA‑approved forms, verifying veteran and income status with applicable agencies.
• Conduct housing and comprehensive supportive services assessments and develop a Family Services Plan.
• Provide service coordination between the program and community partners for persons served and their families/supporters.
• Collaborate with persons served, family members, housing agencies, and professionals to revise service plans and develop placement, stabilization, and follow‑up plans.
• Arrange and support housing and wraparound services, connecting participants to eligible VA and mainstream resources for transition to or maintenance of permanent housing.
• Travel to housing sites to support persons served, monitor progress, and address health, behavioral, or life‑skills issues.
• Communicate effectively (within confidentiality rules) with clients and service teams to rapidly address and correct service or support issues.
• Ensure appropriate use and documentation of any temporary financial assistance.
• Support rapid re‑housing, promote healthy decision‑making, secure financial independence, and promote relationships within the community.
• Reassess participant categories according to SSVF requirements.
• Participate in program and professional supervision, attend staff and Project Management Team meetings and trainings, and contribute to team‑based collaborative planning.
• Complete reporting in HMIS and other required systems.
• Complete other duties as needed and requested.
