Job summary

Volunteers of America Chesapeake & Carolinas is seeking a Case Manager to provide day-to-day support for participating veteran households within the SSVF program. You will help with intake, assessments, and linking clients to housing and wraparound services.

Based in NC regional offices (Durham, Fayetteville, Rocky Mount, or Salisbury), the role requires travel to VA and community partners and collaboration with clinical and housing teams.

Qualifications

• Bachelor's degree in counseling psychology, rehabilitative counseling, or related human fields.

Responsibilities

• Coordinate outreach for the SSVF program and visit VA, shelters, and other locations.
• Screen program participants for eligibility using VA-approved forms.
• Conduct housing and comprehensive supportive services assessments and develop a Family Services Plan.
• Provide service coordination between the program and community partners.
• Travel to housing sites to support participants and address health or life skills issues.
• Ensure timely HMIS reporting and documentation.
