The position emphasizes trauma-informed, person-centered, culturally responsive, harm-reduction practices with flexible work-from-home options

Responsibilities

• Provides intensive, housing-focused case management to Veterans experiencing homelessness or at risk for homelessness.
• Maintains caseload of up to four active Veterans with frequent contact and personalized support.
• Develops and updates individualized service plans (ISPs) and coordinates housing navigation and supportive services.
• Acts as a consistent point of contact and advocate using trauma-informed, person-centered, harm-reduction approaches.
