Job summary

Veterans Place of Washington Boulevard Inc. is seeking an Assistant Case Manager to provide on-site direct care for veterans in a two-year Transitional Housing program. You will assist with transportation, administer supportive activities, and help residents re-enter society as self-sufficient individuals.

The role requires flexibility to cover various shifts, strong interpersonal skills, and commitment to privacy and safety standards within a CARF-accredited setting.

Qualifications

• Experience working with the public or in a customer service capacity preferred.
• Ability to relate well to others and handle administrative responsibilities.
• Must have a valid driver's license.
• Veteran status preferred.

Responsibilities

• Operate as on-site direct care worker for Veterans Place Transitional Housing program.
• Provide transportation/van driving for residents as needed.
• Develop and implement programs and community activities to support residents.
• Maintain accurate documentation of interactions, shift activities, reports, and inspections.
• Ensure safety and security of residents, staff and facility.
• Communicate policies and concerns to Milieu Coordinator and Case Managers.
• Assist with room turnovers, inspections, and maintenance reporting.
• Follow privacy, ethics, and regulatory standards in service delivery.
