Qualifications

• Master’s Degree in Social Work, Psychology, Counseling, or related field.
• Two years of experience in social services or related field preferred.
• Experience working with homeless and/or veterans preferred.
• Certification in Substance Abuse Counseling preferred.
• Strong oral and written communication skills.
• Ability to work independently and within a team.

Responsibilities

• Assesses veterans referred to the program for housing, employment, education, financial, and medical needs.
• Coordinates care with VA personnel and community partners for eligibility and health services.
• Facilitates intake, compiles client files, and ensures complete eligibility documentation.
• Creates and monitors Individual Action Plans (IAP) with clients.
• Provides guidance to Case Manager I & II staff on client IAPs and intervention strategies.
• Maintains confidential client files in accordance with HIPAA and organizational standards.
• Performs home visits to provide case management services and ensure health and welfare of clients.
• Conducts crisis intervention and supports staff during program operations.
