Qualifications

Responsibilities

• Assess veterans\' problems in housing, employment, education/training, finances, sobriety, legal, family/social, and medical/psychiatric.
• Coordinate care with VA personnel and other community partners for eligibility documentation and health services.
• Facilitate client intake and compile files with complete eligibility documents.
• Collaborate with clients to develop Individual Action Plans (IAP).
• Maintain confidential client files per organization standards and ensure documents are complete and properly filed.
• Meet with clients as required and document progress notes in DAP format.
• Review and update IAPs monthly and prepare discharge summaries.
• Conduct home visits to provide case management services and ensure client welfare.
• Assist in quality management, data collection, and reporting.
• Facilitate life skills classes and provide services to veteran subpopulations.
• Conduct drug and alcohol screenings as needed.
• Maintain HIPAA confidentiality and accurate HMIS data entry.
• Document client referrals and participate in team meetings and trainings.
• Maintain time sheets and other required documentation accurately.
• May drive company vehicle for client transportation or work activities.
• Perform other duties as required.
• For programs with housing/SSVF or workforce components, provide targeted case management and employment support.
