Job summary

U.S.VETS Phoenix seeks a Case Manager II to provide trauma-informed, strengths-based case management to veterans facing housing instability, trauma, and other barriers to stability. You will assess needs and develop individualized service plans addressing housing, employment, education, recovery, and health challenges.

You will coordinate with the VA, landlords, employers, treatment providers, and community resources; conduct home visits; and support group work for diverse veteran populations,

Qualifications

  • Bachelor's degree in social work, psychology, counseling, or related field.
  • At least two years of work experience in case management or related social services.
  • Ability to pass background checks for programs involving incarcerated or recently released clients.
  • Strong communication, documentation, and organizational skills.
  • Proficiency in Microsoft Office and electronic records systems.
  • Valid driver's license with clean driving record; able to complete driver training.
  • Experience with homeless, veteran, or justice-involved populations (preferred).
  • Certification in Substance Abuse Counseling (e.g., CCAPP, CAADE).
  • Experience facilitating groups or life skills classes.

  • Support reporting, audits, and file reviews in collaboration with the Program Coordinator and Director of Behavioral Health.

U.S. VETS the nation’s largest nonprofit dedicated to ending veteran homelessness—is hiring a Case Manager II to provide comprehensive support to veterans overcoming housing instability, trauma, mental health challenges, and other barriers to long‑term stability.

What You’ll Do

As a Case Manager II, you will provide trauma-informed, strengths-based case management to veterans Responsibilities include:

  • Conduct thorough client assessments and develop individualized service plans (IAPs) addressing housing, employment, education, recovery, legal, financial, medical, and mental health needs.
  • Coordinate and monitor services with the VA, landlords, employers, treatment providers, and community resources to help clients meet their goals.
  • Provide regular one‑on‑one meetings with clients and conduct home visits (project-based and scattered‑site housing).
  • Conduct home visits at project‑based units or scattered sites to ensure health and welfare of clients
  • Transport clients in company vehicles to appointments, interviews, court appearances and other meetings as needed.
  • Facilitate life skills groups and/or therapeutic groups tailored to specialized veteran populations (e.g., female veterans, justice‑involved veterans, families with children).
  • Assist with crisis intervention, urgent client matters, and after‑hours support as needed.
  • Document all services and critical incident reports in a timely and professional fashion using MCR, and other electronic client record systems.
  • Conduct random drug and alcohol screenings and ensure compliance with program rules and service plans.

QualificationsRequired Qualifications
  • Bachelor's degree in social work, psychology, counseling, or related field
  • At least two years of work experience in case management or other roles in the social services arena
  • Ability to pass background checks if assigned to programs involving incarcerated or recently released clients
  • Strong communication, documentation, and organizational skills
  • Proficiency in Microsoft Office and electronic records systems
  • Valid and current driver's license with clean driving record to meet company insurance requirements; ability to complete and pass a driver training course
  • Not required but a definite PLUS!
  • Experience working with homeless, veteran or justice‑involved populations
  • Certification in Substance Abuse Counseling (e.g., CCAPP, CAADE)
  • Experience facilitating groups or life skills classes
