Qualifikationen

• Graduation from high school or equivalency is required.
• Bachelor's degree in social work, psychology, counseling, sociology, rehabilitation services, human services, criminal justice, nursing, public administration, or closely related field is preferred.
• Experience working with Veterans, military families, behavioral health populations, or related fields is preferred.
• Incumbent must communicate effectively in spoken and written English.

Aufgaben

• Conduct needs assessments for Veterans and families and identify barriers to care.
• Assist Veterans in accessing VA healthcare, housing resources, employment services, and transportation supports.
• Coordinate referrals and warm handoffs to internal and external service providers.
• Collaborate with Veteran organizations, County Veteran Service Officers, VA representatives, healthcare providers, housing agencies, workforce programs, and community partners.
• Conduct follow-up contacts to ensure service linkage and re-engagement as needed.
• Provide crisis support coordination and information to family members when appropriate.
• Document services in the electronic health record promptly and keep records updated.
• Maintain productivity to meet program goals through direct contact and outreach.
