Job summary

Coastal-Behavioral-Health is seeking a Case Manager to coordinate admissions and after-care for service members, veterans and dependents, guided by clinicians and individualized treatment plans.

You will build connections with military and community resources, refer as needed, and support discharge planning while collaborating with the Director of Military Programs and other professionals to advocate for the patient’s ongoing care and independence.

Qualifications

• Bachelor’s degree in Human Services with 2+ years in healthcare.
• Lead generation, referral source development, and account maintenance.
• Extensive knowledge of military culture and independence in work.
• Excellent reading, communication (written and verbal) and presentation skills.

Responsibilities

• Coordinate admissions and discharge processes for the Military program.
• Link patients to behavioral health, ASAP, CAFAC resources; facilitate referrals.
• Collaborate with clinicians, service members, bases, and community agencies.
• Advocate for the patient as a member of a multidisciplinary team.
