Job summary

Centerstone in Florida seeks a Veteran Care Navigator to provide in-person veteran peer support and educate the community about Trauma Recovery Center programs. Qualifications

Bachelor's degree in human services field or 5 years of experience in the behavioral health field.
Military Veteran status required.
Willing and able to obtain the certification upon hire.

Responsibilities

Provides services meeting quality standards and in compliance with various regulatory bodies and KPIs.
Represents Centerstone to internal and external stakeholders; develops Trauma Recovery Services programming; provides community education and consultation as needed.
Provides targeted case management and referrals for trauma-based counseling services with providers of the Trauma Recovery Center, using shared-decision making to ensure individualized care.
Maintains documentation and supports coordination across community-based behavioral health care settings, including with local VA health providers.
