Job summary

The Salvation Army Midland Division Veteran's Residence in Saint Louis, MO seeks a Transition in Place Case Manager to supervise and coordinate engagement in assisting veterans with transitional support. This full-time role encompasses 40 hours per week with a salary of $44,000 per year.

Core duties include intake and assessment, service planning, progress notes, and collaboration with Housing Specialist to ensure safe housing and successful transitions for veterans.

Qualifications

• Bachelor's degree in human services or a related field plus 1 year of experience.
• Experience with recovering substance abusers and veterans is a plus.
• CADC/RASAC/LPC/LCSW certification is a plus.
• Strong interpersonal and communication skills; able to work with homeless and veteran populations.

Responsibilities

• Complete case management functions including intakes, assessments, service plans, progress notes, and discharge summaries.
• Maintain documentation in professional language and HMIS databases as required.
• Conduct home visits with Housing Specialist to ensure safe living environment.
• Provide daily, weekly, monthly, and quarterly outcome measurements and report progress to supervisor.
• Coordinate outreach, admission, and discharge with Program Director and related services.
• Lead weekly educational sessions on life skills, employment skills, and available resources.
• Maintain client files with up-to-date information and ensure confidentiality.
• Participate in staff meetings, supervision, and mandatory trainings.
