ADP, Inc. in Philadelphia, PA is seeking a Case Manager for the SSG Fox Suicide Prevention Grant Program.

The role provides veteran-centered support to service members and veterans at risk, focusing on engagement, safety planning, and coordinated care to promote recovery and connection to services. You will assess eligibility, develop individualized care plans, coordinate referrals to VA and community resources, document progress, and participate in team meetings.

Qualifications

• Bachelor's Degree preferred; equivalent education and experience considered.
• Minimum of 2 years of experience working with veterans or high-risk populations.
• Engage diverse populations with empathy and professionalism.
• Strong organizational, documentation, and communication skills.
• Experience with U.S. Armed Forces veterans is a plus.
• Excellent communication; able to work independently.
• Valid driver's license with reliable vehicle.

Responsibilities

• Conduct eligibility assessments and intakes per grant requirements.
• Provide individualized guidance, coaching, and support.
• Maintain client confidentiality.
• Develop and implement case management and safety plans with veterans.
• Provide crisis intervention and safety planning per VA/VMC policy.
• Facilitate referrals to VA services, housing, employment, and other resources.
• Maintain regular participant contact and monitor progress using motivational approaches.
• Document interactions per VA and VMC standards.
• Participate in weekly case conferences and multidisciplinary meetings.
• Coordinate outreach and veteran engagement events.
• Support data collection for grant metrics (PHQ-9, ISEL-12, etc.).
• Provide budgeting and life skills education to participants.
• Link participants to job training, credit counseling, or similar services.
• Advocate across systems for participants’ rights and reduce stigma.
• Provide direct assistance (transportation, forms, calls).
• Uphold confidentiality and HIPAA/VA/VMC standards.
