Job summary

ADP, Inc. in Reading, PA is seeking a Case Manager (Veteran Services) to provide trauma-informed advocacy and comprehensive case management for a portfolio of 15–30 Veteran households, guiding housing stability from crisis to permanence.

You will collaborate with interdisciplinary teams, assess eligibility, develop individualized plans, coordinate resources, ensure regulatory compliance, and outreach to connect veterans with VA benefits and other public supports.

Qualifications

• Bachelor’s degree or equivalent work experience in social services or related fields.
• Minimum two years in non-profit or social services program delivery.
• Experience with grant-funded programs, especially veteran housing services.
• Proficient with databases and Microsoft Word; strong organizational skills.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Maintain a caseload and provide housing-first case management for Veteran households.
• Assess client eligibility and service needs; develop individualized housing stability plans.
• Coordinate with interdisciplinary teams to implement program resources.
• Ensure case files comply with confidentiality and grant requirements.
• Outreach to identify eligible veterans and connect them to VA benefits.
• Develop relationships to generate referrals and promote agency services.
• Attend internal and external meetings and perform other duties as assigned.
