Job summary

Universal Health Services seeks a Case Manager for the Military & Veteran Support Program to coordinate comprehensive case management services for active-duty service members, veterans, and other eligible military-connected patients receiving behavioral health and/or substance use treatment.

You will serve as a liaison among patients, treatment teams, military resources, referral sources, and community providers, focusing on continuity of care and accessing necessary resources for successful

Qualifications

Bachelor's degree in social work, psychology, counseling, human services, or related field required.
Master's degree in social work, counseling, psychology, or related behavioral-health discipline preferred.

Responsibilities

Coordinate comprehensive case management for military-connected patients receiving behavioral health or substance use treatment.
Serve as liaison among patient, treatment team, family/support system, and community resources.
Coordinate discharge planning and aftercare across multiple services and settings.
Engage patients and family in goal setting, care planning, and transition back to the community.
Communicate effectively with referral sources, partners, and interdisciplinary team.
