POSITION SUMMARY:

The Case Manager for the Military & Veteran Support Program is responsible for coordinating comprehensive case management services for active-duty service members, veterans, and other eligible military-connected patients receiving behavioral health and/or substance use treatment.

The Case Manager serves as a key liaison among the patient, treatment team, family or identified support system, military and veteran resources, referral sources, payors, and community providers. The position focuses on ensuring continuity of care, reducing barriers to successful treatment and discharge, and connecting patients with appropriate clinical, military, veteran, and community-based resources.

The Case Manager works collaboratively with the interdisciplinary treatment team while demonstrating an understanding of military culture, confidentiality, professional boundaries, and the unique psychosocial needs that may affect military-connected individuals and their families.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Case Management & Care Coordination

• Complete timely case management assessments and identify psychosocial, environmental, financial, occupational, family, transportation, housing, and other barriers that may affect treatment or discharge.
• Develop and maintain an individualized case management plan based on identified patient needs.
• Participate in interdisciplinary treatment planning and communicate relevant barriers, resources, and discharge needs to the treatment team.
• Coordinate services throughout the patient’s episode of care to support continuity and progression toward discharge goals.
• Assist patients in navigating appropriate military, veteran, behavioral health, substance use, medical, and community resources.
• Facilitate referrals and confirm appropriate follow-up services prior to discharge whenever possible.
• Maintain timely, accurate, and clinically appropriate documentation of case management interventions.

Military & Veteran Resource Coordination

• Demonstrate sensitivity to military culture, including the potential impact of deployment, reintegration, occupational demands, family separation, transitions from military service, and service-related stressors.
• Develop working relationships with appropriate military, veteran, and community-based referral partners.
• Assist patients with identifying appropriate resources based on their individual circumstances and eligibility.
• Coordinate with approved military or veteran contacts when clinically appropriate, authorized, and consistent with applicable confidentiality requirements.
• Maintain professional communication with referral partners and external professionals throughout the continuum of care.

Discharge & Aftercare Planning

• Begin discharge planning early in the patient’s treatment episode and continually reassess discharge needs.
• Collaborate with the patient and interdisciplinary team to develop a realistic and comprehensive aftercare plan.
• Coordinate outpatient behavioral health, substance use, medication management, medical, peer-support, housing, transportation, employment, and other resources according to identified patient needs.
• Identify potential barriers to successful discharge and elevate unresolved barriers to clinical leadership as appropriate.
• Ensure discharge resources and follow-up information are accurately documented and communicated.

Patient & Family Engagement

• Engage patients in identifying goals, strengths, support systems, and anticipated challenges following discharge.
• Involve family members, significant others, or other identified supports when clinically appropriate and properly authorized.
• Provide education regarding available community, military, veteran, and recovery-support resources within the Case Manager’s scope of practice.
• Promote patient self-advocacy, independence, resilience, and successful transition back to the community.

Communication & Collaboration

• Maintain professional, timely, and responsive communication with internal and external stakeholders.
• Collaborate effectively with therapists, nursing, physicians/providers, utilization management, admissions/business development, and other members of the interdisciplinary team.
• Respond promptly to inquiries from authorized referral sources, community partners, military contacts, payors, and other professionals.
• Escalate significant barriers, safety concerns, or continuity-of-care concerns to the appropriate clinical leader.
• Participate in treatment team meetings, case conferences, and program-specific meetings as assigned.

QUALIFICATIONS:

Education:

• Bachelor's degree in social work, Psychology, Counseling, Human Services, Behavioral Health, or a related field required.
• Master's degree in social work, Counseling, Psychology, or a related behavioral-health discipline preferred.

Experience

• Previous case management, behavioral health, substance use treatment, healthcare, or social services experience preferred.
• Experience working with military service members, veterans, military families, or military-affiliated organizations is strongly preferred.
• Experience providing discharge planning and coordinating community resources preferred.

Licensure/Certification:

• Current North Carolina Driver’s License
• CPI Certification (Can be obtained after hire)

Knowledge and Skills:
