Job summary

Universal Health Services, Inc. is seeking a Case Manager for the Military & Veteran Support Program to coordinate comprehensive case management services for active-duty service members, veterans, and other eligible military-connected patients receiving behavioral health and/or substance use treatment.

Qualifications

• Bachelor's degree in social work, psychology, counseling, human services, or related field required.
• Master's degree in social work, counseling, psychology, or related behavioral-health discipline preferred.
• Experience in case management, behavioral health, substance use treatment, healthcare, or social services preferred.
• Experience working with military service members, veterans, military families, or military-affiliated organizations preferred.
• Current North Carolina Driver’s License.
• CPI Certification (can be obtained after hire).

Responsibilities

• Coordinate comprehensive case management and discharge planning for military-connected patients.
• Serve as liaison among patients, treatment team, families, and external resources.
• Ensure continuity of care and connect patients with military, veteran, and community resources.
• Collaborate with interdisciplinary team to address barriers and discharge needs.
• Maintain accurate documentation of interventions and communication.
