Job summary

USVETS Prescott is seeking a Case Manager I to guide veterans toward independence under supervision by the Director of Behavioral Health. The role coordinates services across housing, employment, health, and legal needs, and maintains confidential files.

You will conduct intake, develop and monitor action plans, document progress in DAP, perform home visits, and assist with life skills groups. Strong communication and ability to work with diverse clients are essential.

Qualifications

  • High School Diploma required.
  • One year of experience in social services or related field preferred.
  • Experience with homeless and/or veterans preferred.
  • Certification in Substance Abuse Counseling preferred.
  • Experience facilitating support groups preferred.

Responsibilities

  • Assess veterans referred to the program across housing, employment, education/training, financial, health, and legal needs.
  • Coordinate care with VA and community partners for eligibility and services.
  • Facilitate intake and compile client files with complete eligibility documentation.
  • Develop and monitor Individual Action Plans (IAP) approved by supervisor.
  • Maintain confidential client files and ensure documents are properly filed and signed.
  • Document progress in DAP format and update IAPs monthly.
  • Conduct home visits to provide case management and welfare checks.
  • Assist with life skills classes and group services for veteran subpopulations.
  • Enter data into HMIS and other required databases.

Job description

This position reports directly to the Director of Behavioral Health at U.S.VETS. With supervision, the Case Manager I is responsible for providing guidance to veterans to achieve their highest level of independence. Case management is a collaborative process which assesses, plans, implements, coordinates, monitors, and evaluates the selection of services to optimally meet each individual clients’ needs.ResponsibilitiesFor Case Managers of all U.S.VETS Programs:Under supervision of Case Manager III, Program Coordinator, or Clinical Director, assists with assessing veterans referred to the program to include any problems, needs, or barriers identified in the following areas: Housing, Employment, Education/Training, Financial, Sobriety Support/Addiction Treatment, Legal, Family/Social, and Medical/PsychiatricCoordinates care with VA personnel and other community partners on behalf of clients to for eligibility documentation, mental and physical health servicesFacilitates client intake process and compiles client files for the program and teammates, ensuring all program eligibility documentation is completeWith supervision, collaborates with clients to complete Individual Action Plans (IAP) that are approved and signed by Case Manager III, Program Coordinator, or Clinical DirectorAssists all Case Managers in maintaining confidential client files in a professional and timely manner ensuring all forms are the most recently revised versions and in compliance with organization standards; all required documents are present, complete, signed by the client and staff as applicable; and documents are placed in the correct order according to organization standardsMeets with clients as specified by program requirements and documents progress notes in Data/Assessment/Plan (DAP) format that are approved and signed by Case Manager III, Program Coordinator, or Clinical DirectorReviews and documents updates for IAPs on a monthly basisCompletes timely discharge summaries that are approved and signed by Case Manager III, Program Coordinator, or Clinical DirectorConducts home visits (whether at project-based units or scattered site) to provide case management services and ensure health and welfare of clientsAssists in quality management activities, data collection, and preparing reportsWith supervision, facilitates client life skills classesAssists in providing specialized case management services and life skills groups/classes to veteran subpopulations as applicable (such as female veterans, disabled veterans, Iraq/Afghanistan veterans, etc.)Conducts random drug and alcohol screenings of clientsMaintains confidentiality in compliance with the Health Insurance Portability and Accountability Act (HIPAA) standardsCompletes accurate and timely data entry into the HMIS system as applicable and other required internal and external databasesDocuments client referrals for servicesAttends scheduled team meetings and trainingsMaintains and submits time sheets, mileage logs, client expenses, and other required documentation accurately and in a timely fashionPerforms other duties as requiredStaff who have a valid driver’s license and meet company insurance requirements may be requested to complete a provided driver training course and drive company vehicles for work-related activities to include client transportationRequirementsHigh School Diploma requiredOne year of experience in social services or related field preferredExperience working with homeless and/or veterans preferredCertification in Substance Abuse Counseling preferredExperience facilitating support groups preferredStrong oral and written communication skillsProfessional manner and appearanceAbility to work independently and within a teamAbility to take directionAbility to work effectively with diverse group of clients, staff, and community membersComputer proficient in Microsoft Office and InternetValid driver’s license required.
