Qualifications

• Bachelor’s degree in Human Services and at least two years of healthcare experience.
• Ability to focus on lead generation, referral development, and maintaining accounts.
• Extensive knowledge of military culture and systems is required.
• Ability to perform with minimal supervision and exercise independent judgment.
• Strong reading, written and verbal communication skills are essential.

Responsibilities

• Coordinate admissions and after-care needs based on clinicians’ recommendations and patient goals.
• Familiarize with community and military resources to support access to services.
• Complete admission and discharge processes for the Military program.
• Communicate with military bases and referral sources to ensure timely documentation and follow-up.
• Collaborate with clinicians and community agencies to link patients to continuing care resources.
• Act as a member of a multidisciplinary team and advocate for the patient.
