Job summary

New England Center and Home for Veterans (NECHV) seeks a dedicated Case Manager to coordinate services for Veterans enrolled in the SSG Fox Suicide Prevention Grant Program. You will manage a caseload, complete ISPs, assessments, and case notes, and connect Veterans with VA and civilian benefits.

This role requires a bachelor’s degree and experience in case management, trauma-informed care, crisis intervention, and navigating community resources.

Qualifications

• Bachelor’s degree and 1–2 years of experience serving vulnerable populations (or equivalent experience)
• Experience providing case management and navigating community resources
• Strong understanding of local community-based resources and benefits
• Experience with motivational interviewing and trauma-informed care
• Experience with crisis intervention and de-escalation
• Flexible self-starter with strong analytical skills
• Experience supporting individuals at risk of suicide is preferred
• Group facilitation experience is preferred

Responsibilities

• Manage a caseload of Veterans enrolled in the program and coordinate services
• Complete and update ISPs, assessments, screening tools, and case notes
• Connect Veterans with VA and civilian benefits and NECHV programs
• Collaborate with staff, partners, and stakeholders to establish referrals and participate in case conferences
• Consult with Mental Health Clinician for clinical connections
• Engage Peer Support Specialists for navigation and guidance
• Coordinate suicide prevention financial assistance for employment and safety training
• Ensure data collection is complete, accurate, and compliant with requirements
• Attend meetings, trainings, case conferences, and supervision
• Perform other duties as needed
