Job summary

Veteran Pathways of New England Inc is seeking a Suicide Prevention Case Manager to work with Veterans and families, developing service plans and coordinating resources to support safety, housing, and financial stability.

The role involves field-based outreach across Massachusetts, supervising Peer Specialists, and ensuring documentation meets VA program requirements. A strong background in veteran services and crisis response is preferred.

Qualifications

• High school diploma or equivalent with relevant experience in case management, human services, veteran support, outreach, behavioral health, or a closely related field.
• Preferred qualifications include higher education in Social Work or related fields and experience with veterans or behavioral health resources.
• Experience supervising or coordinating other staff is a plus.

Responsibilities

• Conduct participant intake and eligibility screenings, develop service plans, manage caseloads, and perform follow-ups.
• Coordinate referrals to VA and community services for housing, employment, and financial resources.
• Provide day-to-day supervision to Suicide Prevention Outreach Peer Specialists and oversee outreach activities.
• Engage with VA facilities and community partners through events and outreach meetings.
• Document records, ensure program compliance, and maintain confidentiality; travel across MA service area.
