Harmony At Home Senior Care is seeking a Veteran Service Coordinator to help fellow Veterans access VA-funded home care programs. Training will cover VA benefits, documentation, and referral pathways, with emphasis on building trust and meaningful connections.

The role targets Veterans transitioning to civilian life and those pursuing healthcare, outreach, or service coordination, offering growth, mentorship, and a mission-driven environment.

Responsibilities

• Conduct community outreach focused on Veteran populations.

• Build relationships with: Veteran service organizations, VA-related community partners, case managers and Veteran advocates.
• Assist Veterans through the initial steps of the VA home care process.
• Coordinate referrals and admissions with internal care teams.
• Track outreach, referrals, and follow-ups in CRM systems.
