Qualifications

• Bachelor’s degree in social work, human services, psychology, or related field.
• Minimum of three years of case management experience in a social service setting, including at least one year with high-barrier or homeless populations.
• Ability to manage a complex caseload and work independently.
• Familiarity with veteran services, housing programs, and local systems.
• Proficiency with client data tracking systems or relational databases.
• Strong problem-solving, crisis intervention, and advocacy skills.
• Good communication and teamwork abilities.
• Valid driver’s license and reliable transportation.

Responsibilities

• Report directly to the Program Manager.
• Manage a reduced caseload with complex needs, including homelessness and co-occurring challenges.
• Conduct assessments and develop housing/service plans for each Veteran.
• Assist in securing safe, affordable housing and provide navigation support.
• Maintain ongoing contact to monitor progress and provide coaching.
• Collaborate with landlords, housing authorities, and partners to place participants.
• Support access to benefits, employment services, and behavioral health care.
• Complete documentation and data entry accurately.
• Participate in coordinated entry and case conferences.
• Mentor junior case managers and serve as resource for complex cases.
• Assist onboarding and training as assigned.
• Review eligibility, recertifications, and exits per timelines.
• Assist with Temporary Financial Assistance requests when needed.
• Support quality assurance and file review for compliance.
• Represent VMC at community meetings and outreach events.
• Maintain confidentiality and adhere to guidelines.
• Perform other duties as assigned.
