Job summary

WestCare Pacific Islands Inc. is seeking a Health Care Navigator/Case Manager to provide veteran-centered case management and housing stabilization, partnering with VA and community services to promote stability and self-sufficiency.

Applicants should have strong communication, organizational skills and the ability to manage an active caseload, while maintaining confidentiality and ethical standards in a trauma-informed, culturally respectful approach.

Qualifications

• Education: High School Diploma or equivalent; college degree preferred.
• Experience: case management, housing or homeless services helpful.
• Skills: strong communication, organization, problem-solving and cultural humility.
• Ability to manage an active caseload with confidentiality and ethics.

Responsibilities

• Maintain an active caseload of Veterans and families at risk of homelessness.
• Conduct screenings, intake, eligibility and needs assessments.
• Develop and monitor individualized housing stability plans.
• Coordinate referrals and advocate for services (VA, healthcare, benefits, housing).
• Maintain accurate and confidential records and HMIS data entry.
