Qualifications

Responsibilities

• Maintain productivity with regular client contacts and documentation.
• Develop individualized care plans with treatment teams and VA partners.
• Coordinate referrals and housing resources to achieve stable housing.
• Provide advocacy for rent, services, and entitlements as needed.
• Maintain HMIS records and ensure compliance with data standards.
